People were involved in assessments of their needs. Staff reviewed assessments taking account of people’s communication, personal and health needs. Care delivery was based on the latest evidence and recognised best practice. Staff worked collaboratively with other agencies involved in patients’ care to achieve the best possible outcomes and to support smooth transitions between services.

Patients were actively involved in the assessment of their needs. Staff regularly reviewed these assessments, taking into account patients’ communication requirements, as well as their personal and health needs, to ensure care remained appropriate and effective.

We found that consent to care and treatment was discussed and explained to patients.

At our last assessment we rated this key question good. At this inspection the rating remained good. This meant patient’s outcomes were consistently good, and patient’s feedback confirmed this.
