Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. This is the first assessment for this newly registered service. This key question has been rated good. This meant people were safe and protected from avoidable harm.

Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice.

The provider had effective systems in place to support learning from accidents, incidents, complaints and concerns. We reviewed incident and accident records, along with the complaints process, and found these were underpinned by clear policies and procedures that provided staff with appropriate guidance.

Staff described a positive and open culture where concerns could be raised. They told us they felt supported when reporting issues and were kept informed of any actions taken in response. This approach helped to ensure concerns were identified promptly, lessons were learned, and improvements were implemented in a timely manner.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services.

People experienced coordinated care through effective partnership working and timely referrals to external professionals.

The Registered Manager described working collaboratively with GPs, physiotherapists, occupational therapists, community nursing teams, falls services and safeguarding teams to ensure people received appropriate support. Referrals and escalations were completed when required, and people and their relatives were involved in decision-making wherever possible.

The service demonstrated a commitment to partnership working and advocating for people to ensure their needs were understood and responded to effectively.

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately.

People were protected from abuse, neglect and avoidable harm through effective safeguarding systems, clear reporting procedures and robust management oversight. The service operated in line with organisational safeguarding procedures and local authority reporting pathways.

The Registered Manager demonstrated a strong understanding of safeguarding responsibilities and the importance of professional curiosity, observation and listening to both people and staff. They explained, "Being emotionally switched on around the customer and listening to care staff is important. If something feels off, that should be enough to increase your awareness."

Monthly analysis of accidents, incidents and safeguarding concerns was completed to identify trends, learning opportunities and preventative actions. Records demonstrated incidents were reviewed appropriately and actions were implemented to reduce future risks.

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them.

Risks were assessed and managed in a way which promoted both safety and independence. Individual risk assessments reflected people's circumstances, preferences and support needs and were reviewed regularly. Staff demonstrated a clear understanding of balancing risk management with choice and autonomy.

One staff member explained, "Care plans give me information about a person's needs, risks, preferences and the support they require. They help me understand how to manage risks while still promoting the person's independence and choices."

Safe environments

Score: 3

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care.

People were supported in environments where risks were assessed and managed appropriately.

Care records included information regarding mobility, equipment and environmental considerations. Risk assessments reflected people's individual circumstances and identified measures required to support them safely in their own homes.

Staff understood their responsibilities for reporting environmental concerns and equipment faults to ensure risks were addressed promptly.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs.

People were supported by safely recruited staff who possessed the skills, knowledge and experience required to provide safe care.

Recruitment records evidenced appropriate pre-employment checks, including Disclosure and Barring Service checks, references and identity verification.

Staffing arrangements met people's assessed needs and promoted continuity of care using regular staff wherever possible. Relatives spoke positively about the quality of support provided, with one relative describing staff supporting [person] as "professional, friendly and caring."

Training compliance was monitored through audits, supervision and one-to-one meetings. The Registered Manager stated, "When I complete my one-to-ones, I remind staff if they are falling behind on training and agree deadlines with them."

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly.

People were protected through effective infection prevention and control practices.

The Registered Manager demonstrated a good understanding of infection prevention measures, including the appropriate use of personal protective equipment (PPE), hand hygiene and safe working procedures.

One staff member explained, "I use PPE based on the task I am carrying out and the level of risk and follow good hand hygiene practices."

Relatives confirmed staff consistently used appropriate PPE during visits.

Medicines optimisation

Score: 3

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened.

Medicines were managed safely through effective monitoring, auditing and management oversight. Medication Administration Records (MARs) were completed and audited regularly. Audit findings evidenced ongoing monitoring and appropriate follow-up where required.

The Registered Manager demonstrated a good understanding of medicines management, including administration, recording, storage and disposal requirements.
