Leaders demonstrated an understanding of the issues, challenges, and priorities within their service and wider locality. An effective and comprehensive process, including succession planning, was used to identify, understand, monitor, and address current and future risks.

Care provision, Integration and continuity
Leaders explained that the service used technology to enable continual performance monitoring to support the management of health conditions, and facilitate access to care. Staff reported that the service conducted searches to identify vulnerable patients at significant risk of poor health.
Feedback from a local partner was highly positive regarding their collaboration with the practice to support patients. They said that staff listened to their requests and responded promptly and efficiently.
The practice collaborated with other services to meet the needs of its patient population, holding regular multi-agency meetings to discuss and enhance outcomes for those with complex needs. A tailored approach was taken to address community needs, such as encouraging participation in screening programmes. Additionally, the practice participated in local initiatives with neighbouring practices to tackle health inequalities. There were processes in place to ensure people received care and treatment that accounted for their local communities' diverse health and social care needs.

Equity in experiences and outcomes
The figures from the practice’s latest GP national survey showed the practice’s performance was in line with both local and national averages in five of the six indicators monitored. The provider’s internal patient survey results conducted from December 2024 to January 2025, demonstrated a steady improvement in patients’ overall experience at the practice.
Planning for the future
While we did not receive specific feedback from patients regarding the service's future planning, we reviewed 14 detailed and comprehensive do not attempt cardiopulmonary resuscitation records and three case studies. These showed that clinicians discuss decisions with relevant parties, assess patients' mental capacity, and support individuals in making informed choices about their care and future planning where possible.
The practice’s business plan served as a roadmap for improving outcomes and ensuring stability. It focuses on securing necessary resources and aligning staff toward shared objectives, including the practice’s goals to enhance patient experience. The practice identified patients requiring additional support during their registration process and took proactive steps to meet their needs.
Additionally, the provider has signed up to participate in a new project launching in spring 2025, aimed at increasing engagement with Gypsy, Roma, and traveller communities across the borough. The provider also attended local primary care network meetings to discuss strategies for managing conditions such as diabetes and atrial fibrillation.
