Independence, choice and control
Patients told us they felt supported in managing their health and well-being and were treated with respect and dignity. As part of our assessment, we reviewed the practice’s latest national GP patient survey data, which showed that:
- 86% of respondents felt they were involved as much as they wanted to be in decisions about their care and treatment during their last general practice appointment.
- 79% of patients felt their needs were met during their last appointment.
- 88% felt the healthcare professional they saw had all the necessary information about them.
The service also collected regular feedback, which showed a monthly increase in positive outcomes. Patients consistently reported that the staff they interacted with were helpful, compassionate, and supportive.
The provider told us they provided patients with information and guidance to enable patients to make informed decisions about their care.
Staff are fully committed to working in partnership with their patients and empowering them to be active participants in their care. They recognise the importance of patients having access to advocacy and support networks within the community. To facilitate this, the provider has developed strong links with other organisations and supports staff in connecting patients to these resources where appropriate. This approach helped patients maintain their independence as much as possible, and staff demonstrated a clear understanding of how vital this is for the individuals they care for.
The provider has established a suite of policies and procedures, such as a guide to writing effective care plans and a good practice guide, to govern their processes and ensure safe, consistent operations. Additionally, the service was equipped with a range of adapted equipment to support patients, including interpreters, information in foreign languages, and hearing loops for patients with hearing difficulties. The practice had also adopted the NHS chunk and check method to provide information to patients with low literacy rates, to support them to understand and engage with the information provided.
