How staff, teams and services work together
We did not receive specific examples from patients about how staff, teams and services work together. However, we did receive positive feedback regarding the efficiency with which the practice referred patients to other services.
Staff told us they were proud of how practice staff worked well together as a team to meet the needs of their patients.
Partners such as the care home aligned with the practice told us they felt they worked well with the practice as a team. They reported that the staff responded promptly to queries raised by telephone or email. Additionally, medicine management issues were coordinated by the lead GP and resolved quickly. The lead GP maintained regular contact and had recently established monthly reviews to further enhance collaboration.
There were systems in place for sharing information with staff and other agencies to ensure the delivery of safe care and treatment.
Practice staff attended monthly meetings with other teams and services to discuss and coordinate care. For older patients with more complex needs, appropriate referral pathways were in place to ensure their needs were assessed effectively and in a timely manner.
