The property was owned and managed by NHS property services. The premises had disability access. The staff detected and controlled potential risks in the environment and liaised with NHS property services. NHS property services had completed a fire risk assessment in August 2024; the practice manager told us any actions in the fire risk assessment had been implemented.

The staff made sure equipment, facilities and technology were maintained and supported the delivery of safe care. To ensure the continuity of the service, leaders had a business continuity plan which was regularly reviewed. However, the annual calibration of equipment was last carried out in May 2025.

The leaders could not demonstrate that safe recruitment procedures were always being followed. For example, we reviewed 5 staff personnel files and found one staff had not had a Disclosure and Barring service check, another was a clinician and had a basic check instead of the recommended enhanced check. Two staff members did not have a job description or evidence of history or past work (application form or Curriculum Vitae). Two staff did not have verification of their address or references. The practice did not carry out regular checks of clinician’s professional registrations or have a scope of practice and formal supervision in place for the advanced nurse practitioner to ensure staff were working within their agreed competencies.

The service made sure there were enough qualified staff to meet patient needs. Staff had completed the necessary mandatory and role specific training, with the exception of 5 staff where safeguarding training was either not completed or not completed to the correct level. During the site visit we saw evidence of staff annual appraisals.

The staff effectively assessed and managed infection risks. They promptly identified and controlled potential spread, sharing concerns with relevant agencies. A designated infection prevention and control lead was in place, and all staff received appropriate training. Cleaning schedules were followed, and regular audits and risk assessments were completed with actions taken to reduce risks. However, the practice did not have a record of staff immunisation in accordance with the NHS Immunisation against infectious disease green book.
