Supporting people to live healthier lives

Score: 3

The 2025 National GP Patient Survey results showed people were informed about their care and treatment and what the next steps were. Eighty one percent of patients knew what the next step would be after contacting the practice. This was in line with the national and local average results. Since the inspection, GPPS have published new results in July 2026. The new data suggests a slight improvement with 85% of people responding positively.

Staff supported people to manage their health and wellbeing. The practices local primary care network (PCN) supported the practice and carried out an NHS campaign to increase the uptake of health checks.

Staff identified and responded to health risks such as end-of-life needs, emerging long-term conditions, carer responsibilities and supported national health priorities, including smoking cessation and obesity reduction. Staff referred patients to a social prescriber, the local gym and a dietician. The leaders told us they had almost met their target for over 40 years health check for the year 2025 to 2026.

Childhood immunisation uptake for 2024/25 was also below the World Health Organization (WHO) target. The practice provided unverified data that demonstrated they had made improvements and had achieved the WHO target of 90% for all but one area in 2025/26.

The practice submitted unverified data showing maximum achievement for atrial fibrillation, asthma, secondary prevention of coronary heart disease, chronic obstructive pulmonary disease (COPD), heart failure, and stroke/transient ischaemic attack (TIA). Achievement for diabetes and hypertension was slightly below maximum levels.

The practice also submitted unverified data from Interface Clinical Services relating to asthma, diabetes, and chronic kidney disease. Analysis highlighted potential concerns regarding diabetes management, asthma safety indicators, and chronic kidney disease monitoring.

In addition, the practice provided two audits, including a completed two-cycle audit of patients prescribed sodium valproate.
