Well-led
We looked for evidence service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture. At our last assessment, we rated this key question as good At this assessment, the rating remains the same.

The practice had a mission statement which included quality and safety, respect, patient engagement, disease prevention, learning and improving and financial security, which we were told was reviewed in practice meetings. Staff and leaders understood the challenges and the needs of people and their communities.
Staff we spoke to told us they were supportive of and supported by the leadership and management team, the culture and direction of the service, and each other. Staff told us leaders were approachable and took the time to listen and there was a no-blame culture. There was an appropriate whistleblowing procedure, which staff understood and could access.

Governance, management and sustainability
Staff had clear roles, responsibilities, and governance systems to support high-quality, sustainable care.
All staff we spoke with were clear on their individual roles and responsibilities. Leaders proactively supported staff and regularly met with them to complete appraisals and performance reviews. The provider had established governance processes in relation to the quality statements we reviewed during this assessment, which were mostly appropriate for their service.
Staff could access all required policies and procedures. Leaders held regular meetings with staff, during which they discussed emerging risks and potential improvements to the service. We reviewed administration and clinical meeting minutes, but minutes of leadership meetings were not recorded.
Staff took confidentiality and information security seriously, which included verifying people’s identity during remote consultations. Staff submitted data and notifications to external agencies as required.
