The property was owned and managed by NHS property services and shared with other NHS services. At the time of the assessment, refurbishment works were underway; staff had identified and managed associated environmental risks and liaised appropriately with NHS Property Services.

A fire risk assessment completed by NHS Property Services in February 2025 identified actions that, according to the practice manager, had been implemented. Most staff had completed their fire safety training.

Staff ensured equipment, facilities and technology were properly maintained to support the delivery of safe care. A business continuity plan was in place to support service resilience and had last been reviewed in June 2026.

The leaders followed safe recruitment procedures. Recruitment was carried out by the providers human resource team. The practice manager was involved in reviewing the applications and interviewing, all recruitment records were held centrally by the human resource team. During the site visit, we reviewed 3 staff files and found all the required documents checks were in place. Any clinical appointments were reviewed by the clinical lead.

Workforce planning was developed centrally by the provider and reflected the number of registered patients.

The leaders had some systems in place to demonstrate staff had the necessary skills, knowledge, and experience for their roles. Most staff had completed the necessary mandatory training, except for General Data Protection Regulation (GDPR) where 5 staff out of 16 had not completed the training.

Practice management staff told us that clinical staff had consultation reviews. However, at the time of the assessment we were not assured the leaders had a system in place to provide the evidence for assurance that a nursing team staff member was working competently within their scope of practice. Following the site visit we were provided with a draft nurse practitioner scope of practice assurance policy for the relevant staff member which had yet not been ratified by the Clinical Governance Group.

The provider submitted the staff survey for the whole organisation where 21% of staff responded. This showed across the whole organisation 61 % of staff felt they had access to learning and development. In response the provider planned to promote training opportunities across the group and raise awareness of the apprenticeship schemes, access to training grants and the use of study leave to complete mandatory training.

We observed the premises were clean and tidy. The staff effectively assessed and managed infection risks. A designated infection prevention and control lead was in place. Most staff had completed their infection prevention and control training. Cleaning schedules were followed, and regular audits and risk assessments were completed with actions taken to reduce risks.

We reviewed three staff employment records which included a record of staff immunisation in accordance with the NHS Immunisation against infectious disease green book.
