At our last assessment we rated this key question as good. At this assessment the rating has remained as good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and regularly reviewing their health, care, wellbeing and communication needs with them. People and those important to them were involved in the assessment process, and care plans were updated when their needs or circumstances changed. This helped staff understand how to provide care and support that reflected people’s individual needs and preferences.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment in partnership with them, taking account of what was important to each person and the outcomes they wanted to achieve. Care plans reflected people’s assessed needs, preferences and daily routines and were reviewed when their needs or circumstances changed. Staff followed relevant legislation, guidance and current evidence-based practice to support people to receive consistent and effective care.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to identify whether it remained effective and continued to meet their needs. Staff recorded and reviewed people’s health, wellbeing and progress and responded when their needs or circumstances changed. This helped the provider maintain positive and consistent outcomes that reflected both clinical expectations and the outcomes people wanted to achieve.

The provider told people about their rights around consent and respected their decisions when delivering person-centred care and treatment. Staff supported people to understand their options and make their own choices rather than making decisions on their behalf. A staff member told us, “Often when [person] asks for guidance, we say it’s up to [person] and explain [person] has choices.” This demonstrated that staff recognised people’s right to make decisions about their care and daily lives”.
