We looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture.
At our last inspection we rated this key question good. At this inspection the rating changed to requires improvement. This meant the service management and leadership was inconsistent. Leaders and the culture they created did not always support the delivery of high-quality, person-centred care.

Capable, compassionate and inclusive leaders

The service had not had a sonography clinical lead in place for a considerable time, and leaders recognised how this had affected its management. The operations manager had adopted overall responsibility and was able to demonstrate a suitable understanding of the service and its workload, but it was not clear how appropriate and informed strategic decisions could be made for the service in the absence of this clinical lead role.
