At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. People’s needs were assessed prior to them starting to use the service and person-centred care records were in place.Managers also completed the first visits when people started to use the service, to make sure everything was ok for people and staff.

One person told us, “When I started having their care, someone came from the office and did an assessment, [staff member] arranged for me to get [equipment], which I use around the house.” A relative explained, “[Person] had an initial assessment and home visit when we were asked what we needed, the manager was lovely.”

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. People’s care and support needs were detailed in person centred care records. Staff told us they found the records really helpful to guide them on how people wanted their support to be provided. A staff member said, “They (care records) are a big help. They are easy to follow and give you lots of useful information [about the person].”

People told us records were reviewed regularly, and people and their relatives were involved and listened to. They told us any changes were identified and responded to promptly.

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. Staff told us there was good teamwork and open communication between all managers and staff.

Supporting people to live healthier lives

The provider supported people to manage their health and well-being to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support. Staff supported people to manage their health conditions and worked alongside healthcare professionals when specialist advice or intervention was needed.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves. Care and support were reviewed regularly and people told us any changes needed were actioned promptly.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. Staff we spoke with understood their responsibilities and demonstrated how they gained people’s consent. People told us staff always involved them in decisions about their care.
