Responsive
Responsive – this means we looked for evidence that the provider met people’s needs. At our last assessment we rated this key question requires improvement. At this assessment the rating has changed to good. This meant people’s needs were met through good organisation and delivery.

Person-centred Care
The provider made sure people were at the centre of their care and treatment choices. They worked in partnership with people, to decide how to respond to any relevant changes in people’s needs. Care plans had been improved since the last inspection. Due to data sharing limitations they still needed more detailed information, especially about how staff should support people to achieve their goals. The provider was working with partners to improve the quality of the information in care plans.
Care provision, Integration and continuity
The provider understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity. Staff told us communication and teamwork had improved. Care staff and on-site healthcare professionals met daily to share information about people’s care. A staff member said, “We sit together and go through care plans. This has helped with morale.”

Equity in access
The provider made sure people could access the care, support and treatment they needed when they needed it. People could access support at any time of the day or night. The premises and equipment were accessible and suitable to the needs of the service. The provider was working with healthcare partners to improve information sharing to develop holistic care plans.
Equity in experiences and outcomes
Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this. People told us they had achieved good outcomes from their stay at the service. Staff had completed equality and diversity training and understood the importance of treating people equally.
