Assessing needs
We did not look at Assessing needs during this assessment. The score for this quality statement is based on the previous rating for Effective.
Delivering evidence-based care and treatment
The provider did not always plan and deliver people’s care and treatment with them, including what was important and mattered to them. Care plans had been reviewed to ensure they clearly identified people rehabilitation goals. However, detailed information about how staff should support people to achieve their goals was not yet clear. Work was progressing to develop a comprehensive plan with input from all relevant care staff and healthcare professionals. The provider was also implementing an electronic care planning system.

Monitoring and improving outcomes
The provider routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive and consistent, and they met both clinical expectations and the expectations of people themselves. People and relatives told us they had made progress due to the care and dedication of staff. A relative said, “They have got [family member] mobile again. This is through the effort and compassion staff have.”
Consent to care and treatment
We did not look at Consent to care and treatment during this assessment. The score for this quality statement is based on the previous rating for Effective.
