Safe – this means we looked for evidence people were protected from abuse and avoidable harm. This is the first assessment for this newly registered service. This key question has been rated good. This meant people were safe and protected from avoidable harm.

Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice. The provider had systems and processes in place which supported a culture of learning from accidents and incidents.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services. The provider received pre-assessment and referral information which enabled them to ensure there was continuity of care and support for people.

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way for this to be achieved. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately. Policies were in place which ensured people received safe care and support. Members of staff had accessed appropriate training courses to keep people safe. A member of staff told us, "Safeguarding policies are available online and in the safeguarding policies file. If I suspected abuse, I would ensure the person is safe, record all the information about what happened and report it immediately to my manager and follow safeguarding procedures. I understand the importance of confidentiality and whistleblowing."

The provider followed the principles of the Mental Capacity Act 2005 (MCA) to review people’s capacity and understanding around receiving care and support. The MCA provides a legal framework for making particular decisions on behalf of people who may lack the mental capacity to do so for themselves. The MCA requires that, as far as possible, people make their own decisions and are helped to do so when needed. When they lack mental capacity to take particular decisions, any made on their behalf must be in their best interests and as least restrictive as possible.

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs which was safe, supportive and enabled people to do the things which mattered to them. People’s care plans were specific and unique to each individual. Risks were clearly identified along with specific requirements and routines necessary for the provision of safe care and support. A member of staff told us, “I usually access [care plans] through the electronic care system or care files provided by the service. I always check them before supporting someone, so I understand their needs, preferences, and any risks involved.”

Safe environments

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care. The provider ensured equipment was checked and serviced in line with national standards and regulations. Risks associated with the care environment were assessed and mitigated. There was an on-call system in place to support staff who worked outside of normal working hours.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care which met people’s individual needs. Members of staff were recruited safely, and they received training and support to provide care effectively. The provider carried out competency reviews and assessments of tasks and skills required to provide care and support to people. New starters to the service were supported by a comprehensive induction programme.

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. The provider had an infection prevention and control (IPC) policy in place. The use of personal protective equipment (PPE) was detailed in people’s care plans. A member of staff said, “Yes, PPE is always available, including gloves, aprons, masks, and hand sanitiser. We use them properly to protect both staff and service users. I’ve completed infection control training covering handwashing, PPE use, cleaning procedures, waste disposal, and preventing the spread of infections.”

Medicines optimisation

The provider made sure medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened. The provider managed medicines safely. Protocols were in place for ‘as and when’ medicines and they described when, how and why these medicines should be administered. The management team reviewed and audited the administration of medicines. Members of staff were knowledgeable about what to do in the event of a medication error. A member of staff told us, “If a medication error happens, it must be reported immediately to the registered manager, documented correctly, and the person must be monitored to make sure they are safe. The situation should also be reviewed to prevent it happening again."
