Effective – this means we looked for evidence people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence. This is the first assessment for this newly registered service. This key question has been rated good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. People’s needs were assessed appropriately and recorded in detailed care plans. There were good links with people’s families and specialist teams to support reviews of people’s care and support needs.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. The provider had good relationships with specialist health and care professionals. Specialist guidance, for example about eating and drinking, was reflected in people’s care plans and followed by members of staff.

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. There was good communication between teams of staff and key information about people was shared at handover meetings. Members of staff attended team meetings and notes and updates were shared following meetings. This ensured continuity of care and support for people.

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support. People’s care plans continued detailed information about managing their specific health conditions, for example, epilepsy. There was clear information and guidance about what to do in the event of a health event which required intervention and support.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive and consistent and met both clinical expectations and the expectations of people themselves. The provider maintained clear records about hospital admissions and outcomes for people who received respite care and support. There were a range of checks, reviews and monitoring reports in place for people’s health conditions. This enabled the provider to monitor outcomes and share information or take action as required.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. Conversations with people and their representatives took place to ensure people were involved in decisions about their care and support. People’s communication care plans were followed to support their involvement in conversations. A member of staff told us, "I always speak to people and talk them through each task, always gaining consent, for example, ‘We are just going to attach the loops of your sling to the hoist and then we will sit you up, is that ok?’ I always wait for a ‘Yes’ before proceeding and ask them if they are ok throughout and if they are comfortable."
