Well-led
The service was well-led and had visible, approachable and supportive leaders who understood service performance and patient needs. Effective governance arrangements provided clear accountability and oversight of quality, risk and performance, supported by audits, risk registers and timely management information. Staff described an open, inclusive culture where they felt supported to raise concerns, contribute ideas and share learning. The service worked collaboratively with external partners to support continuity of care and used feedback, incidents, audits and external accreditation to drive improvement. Staff were supported through training, professional development and wellbeing arrangements. However, the provider’s vision and values were not fully embedded, several leaders were relatively new in post, and staff awareness of formal freedom to speak up arrangements was limited.
