The service had a positive culture of safety and delivered care in a safe environment with effective systems to identify, manage and learn from risks. Staff reported incidents, near misses and safeguarding concerns appropriately and used learning from investigations, audits and governance processes to improve practice. Patients were cared for by enough suitably trained staff, supported by a structured induction programme and mandatory training compliance of 99%. The service maintained a clean and well-maintained environment, controlled infection risks effectively, ensured equipment was appropriately serviced and safety checked, and managed medicines in line with national guidance. Staff worked closely with partner organisations to maintain safe care pathways and continuity of care, and safeguarding arrangements supported the protection of children and vulnerable adults. However, although staff understood the need to be open and honest when things went wrong, not all staff were able to clearly explain the principles of duty of candour and when it should be applied.
