This meant services were tailored to meet the needs of individuals and delivered to ensure flexibility, choice and continuity of care.

Person-centred Care
The provider made sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in people’s needs. For example, care plans were person centred and included information about people’s needs, choices, and preferences. Staff had a good understanding of people including their likes, dislikes, and things of importance. We observed meaningful relationships and positive interactions between people, staff, and family members.

Equity in experiences and outcomes
Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support, and treatment in response to this. Care plans included information around people’s identity, things which were important to them, their wishes, and relationships they wanted to maintain. Staff ensured people’s social and healthcare needs were fully considered and met. We saw several examples of improved outcomes for people since moving into the home, including one person who previously was very withdrawn and is now engaging with their peers, staff and their family and enjoying going out in the community.
