Person-centred Care

The provider made sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in people’s needs.

We reviewed peoples care plans and daily records staff completed. The records were person-centred and evidenced staff met people’s individual needs. Staff understood the importance of providing care in a person-centred way. One staff member told us, “You can also sign language or write things on paper, it depends on the needs of the client. The support plan also specifies, the care plan is very detailed and is a good prompt”.

Care provision, Integration and continuity

The provider understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity.We found prompt action had been taken when people’s needs had changed, appropriate referrals had been made to ensure people had access to services that could support them. People told us the service acted quickly to support their needs. Comments included, “The changes are put into place quickly” and “Yesterday, the manager rang to run over the process. I had a second call about the plan, that was a courtesy call, they will work round my appointments”.

Providing Information

The provider supplied appropriate, accurate and up-to-date information in formats that were tailored to individual needs.People’s care plans detailed their communication needs and the support they required, this information was regularly reviewed and updated. People and relatives told us the information provided to them suited their individual needs, was available and reviewed regularly. Comments included, “I can go online, and read [Name’s] daily notes if I want to. I don’t feel I need to keep checking”, “All paperwork comes to me, I’m in the loop” and “They talk me through the paperwork”.

Listening to and involving people

The provider made it easy for people to share feedback and ideas, or raise complaints about their care, treatment and support. Staff involved people in decisions about their care and told them what had changed as a result. People and relatives told us they had opportunities to share feedback about the service. Comments included, “I would ring the office and put a complaint to them, but I have nothing to complain about” and “There’s a written complaints form in the folder, or I could email or phone them” and “They have been outstanding”.

Equity in access

The provider made sure that people could access the care, support and treatment they needed when they needed it.The service also ensured people were made aware of their rights and provided people with information on various situations, this included how to raise a complaint about their care.A member of staff told us, “For complaints and compliments, we advise them to phone the office, we give them that number”.

Equity in experiences and outcomes

Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this.Care plans highlighted people’s diverse needs, including their religions and cultures. Staff received equality, diversity and inclusion training. Staff told us people’s diverse needs were met. Comments from staff included, “[Name] can speak through sign language, we know what [Name] is saying, after assessments we go with the manager, to tell us as well so that we get to know how best to support people”.

Planning for the future

People were supported to plan for important life changes, so they could have enough time to make informed decisions about their future, including at the end of their life.At the time of our assessment no one was receiving support at the end of their life.
