Person-centred Care
The provider made sure the person was at the centre of their care which reflected their preferences. The provider respected the decisions of the person they supported, making sure the care was flexible to allow for decisions and plans to be changed if necessary. The provider could call upon staff who were able speak languages other than English which offered more choice and better communication.
Care provision, Integration and continuity
The provider understood the diverse health and care needs of the person they supported and was able to work with other agencies and people in the local community to provide support. The person receiving care could make decisions about important local places within the community they wanted to go to, and staff were able to support them to do this.
Providing Information
Staff were able to communicate easily with the person using either English or their preferred language and they were able to provide information to the person about the services they offered.
The provider had a written contract in place with the person they supported stating the support they offered as well as general useful information about the provider which the person had understood and signed.
Listening to and involving people
The provider made it easy for the person to share feedback and ideas or raise concerns about their support. The provider carried out frequent surveys so the person they supported, and their staff could give feedback which the provider and took on board and acted upon.
Equity in access
The provider made sure that the person could access the care and support they needed when they needed it. They facilitated the person accessing medical services as well as places of worship.
Equity in experiences and outcomes
The provider made sure the person receiving care had equity in outcomes. The person was able to express what they wanted support with and worked with their staff to achieve it. Training about equality and human rights promoted awareness amongst the staff team. Policies were in place to reinforce the providers commitment to ensure the rights of any person they supported now or in the future were upheld.
Planning for the future
Staff discussed future plans with the person they supported. They focussed on ensuring the person had the right care and support in place to enable them to continue to live in their own home in the future.
