We have served a S29 warning notice on Precious Glimpse Roundhay on 28 May 2026, because we had concerns about delivery of safe care and treatment in line with best practice and national guidance, lack of risk assessments for pregnant people using the service, the environment and equipment, infection prevention and control, and lack of systems in place to actively assess, monitor and improve the quality and safety of services.

Well-led
We found new breaches of regulations 12 and 17 and continuing breaches of regulation 17. The culture at the service was not based on speaking up, listening, learning and trust. The registered manager did not demonstrate they understood their roles and responsibilities. They did not have sound oversight of the quality of service being delivered because there were no governance and risk management systems in place. The service did not demonstrate how its focus was on those with protected characteristics under the Equality Act and those from excluded and marginalised groups. The registered manager did not aspire to a culture of continuous improvement and there was no quality improvement programme. However, they knew the service’s local vision and aims.
