We looked for evidence that the service’s leadership, management and governance assured high-quality, person-centred care and promoted an open and fair culture. At our last assessment, we rated this key question as Good. At this assessment, the rating changed to Requires Improvement.

Shared direction and culture

Score: 3

The service had a shared vision, values and culture. This was based on transparency, equity, equality, human rights, diversity, inclusion and engagement. Staff and leaders understood the challenges and the needs of people and their communities.

Staff expressed enjoyment in their roles and valued the collaborative environment. Regular team meetings were held to share updates and promote open communication across the service. Records of meeting minutes and actions were stored and accessible to all staff on the service’s electronic system.

Staff described a friendly and supportive culture, with strong teamwork and a positive working environment. Staff were supported in their professional development and encouraged to keep their skills up to date.

Governance, management and sustainability

Score: 1

The service did not have clear responsibilities, roles, systems of accountability and good governance. The provider had not established governance processes that were appropriate for the service.

The service did not have effective systems and processes embedded to ensure premises-related risks were being managed consistently. The service could not demonstrate how it maintained oversight of its water temperature monitoring and the actioning of out of range recordings. The service did not effectively monitor IPC risks and had not considered its branch site when undertaking its IPC audits for the local integrated care board. There were also gaps in governance and assurance processes, including inconsistent fire safety monitoring records, incomplete cleaning and COSHH documentation, and limited recruitment oversight of a clinical reference.

The provider had also failed to submit 2 statutory notifications to CQC relating to the death of a person who uses its service in line with regulations, demonstrating that its systems for meeting regulatory requirements were not operating effectively. Both deaths had occurred in the preceding 12months and based on our discussions with the service, warranted a statutory notification to CQC.

These findings demonstrated that governance systems were not consistently effective in identifying, monitoring and addressing risks.

However, leaders recorded actions that arose from meetings and shared these with staff. We reviewed meeting records which demonstrated actions were clearly recorded, monitored and followed up. Leaders supported staff and regularly met with them to complete appraisals and performance reviews. Learning from audits and incidents was shared with staff to support continuous improvement. Business continuity arrangements were in place and reviewed annually.
