Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including a menopause support group. This was held virtually and people were able to share experiences as a group. Staff we spoke with were aware of these initiatives and how they applied to the population they supported.

The service had access to a social prescriber and supported people who were referred from the GP, for example, linking people to local services, groups and activities.

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

The service met national targets for childhood immunisation uptake, for example, achieving 92% uptake for children aged 2 receiving their booster immunisation compared to the national average of 90%. The service performed better than the national average for cervical screening among eligible people aged 50 to 64 years, with an uptake rate of 81%. Cervical screening uptake among eligible people aged 25 to 49 years was 78%, which was close to the national target of 80%.

The service completed medication review audits. For example, a recent antibiotic audit showed that all people had received appropriate prescribing as per guidelines. Audit findings were discussed within clinical meetings and used to inform prescribing practice and support continuous improvement.
