We looked for evidence that people and communities had the best possible outcomes because their needs were assessed appropriately. We checked that people’s care, support and treatment reflected these needs and any protected equality characteristics, ensuring people were at the centre of their care. We also looked for evidence that leaders instilled a culture of improvement, where understanding current outcomes and exploring best practice was part of their everyday work.

At our last assessment we did not rate this service as that was not part of the methodology at the time. At this assessment the rating is good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

The service made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.

The transport and care provided would align with trust care plans which were provided to staff. Staff assessed the needs of people throughout the journey.

Staff at the service had access to aids to support with additional communication needs and translation services were in place if required.

Delivering evidence-based care and treatment

The service planned and delivered people's care and treatment with them, including what was important and mattered to them. Staff did this in line with legislation and current evidence-based good practice and standards.

Staff had access to an extensive policy list which was kept up to date. All patient care we observed was in line with current evidence based good practice, for example, we observed good moving and lifting technique.

How staff, teams and services work together

The service worked well across teams and services to support people. Staff made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services.

Whilst on assessment we observed staff worked well together and worked well with staff from other services. In staff feedback we received following the on-site assessment staff also told us they worked well together.

Supporting people to live healthier lives

The service supported people to live healthier lives where possible.

As a patient transport service, the service was not involved in any care beyond the transfer of the patient. Where possible, staff ensured that people were going to be comfortable on the journey and we happy at the location, this included access to water on the journey and following peoples care plans.

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. Patient report forms were monitored as they came through the on the office system.

Each trust had its own key performance indicators (KPIs) regarding timeliness, the service had regular meetings with trusts to discuss KPIs.

The service told people about their legal rights around consent and respected these when delivering person-centred care and treatment.

Staff gained consent prior to treatment was part of the standard process when providing care and treatment. Whilst on the assessment we observed staff obtain verbal consent appropriately.

Leaders assured themselves that staff appropriately sought and recorded patient consent as they reviewed patient record forms.

Staff always reviewed Do Not Attempt Cardiopulmonary Resuscitation (DNACPR) paperwork when they were with people being transported. If they were not completed appropriately staff would raise this with the trust and they would not be considered valid unless appropriately completed.
