Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence.At our last assessment we rated this key question requires improvement. At this assessment the rating has changed to good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, well-being and communication needs with them. The care that people received was in line with their agreed plans. Records showed people’s care plans were reviewed and updated regularly. People, their relatives and professionals were all able to contribute to care plan reviews and regular multi-disciplinary team (MDT) meetings were held to discuss people’s needs.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. People’s care plans included information about their health needs and the service actively engaged with health professionals to ensure this information remained up to date. For example, the service engaged with speech and language services and the GP to ensure that appropriate and proportionate plans were in place for a person they supported.

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. The service worked in partnership with visiting health professionals and any recommendations made about people’s care and support were documented in people’s care plans. A visiting professional told us, ‘Referrals are followed up with good communications. Staff and managers are good advocates for the people they support.’

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support. Regular review meetings were held with health professionals, people and their families to ensure that people’s health needs were met. Where people needed to attend appointments, the provider facilitated this.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves. Staff understood how to record and report changes in people’s health and well-being needs. Where required, records were in place to monitor people’s health needs, for example fluid charts to manage hydration needs.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. People were able to make choices about what they wanted to do and when. Staff understood the importance of ensuring people retained control of their care. A staff member told us, ‘We need to speak to people and we need to listen to them when they tell us what they want.’ Assessments were carried out in line with the Mental Capacity Act, with decisions being made in people’s best interests where necessary.
