At our last review of this key question, it was rated good. At this assessment the rating has remained good. This meant people were safe and protected from avoidable harm.

Learning culture

Score: 3

The provider promoted a positive and open approach to safety. Staff felt comfortable raising concerns, reporting incidents, and learning from them to help improve the service.

People using the service and their relatives knew how to raise concerns or make a complaint and understood the complaints process. One relative said, “If I had any concerns, I would speak to the registered manager. I speak to them when I visit, they are always available.”

The provider had systems in place to deal with complaints and safety concerns. The registered manager reviewed all incidents to identify patterns and take action to reduce risks.

Staff were supported to record and report incidents using clear procedures. The registered manager encouraged open communication through an open-door policy and regular team meetings, where accidents, incidents, and ways to improve the service were discussed.

Safe systems, pathways and transitions

The provider worked with people and healthcare professionals to make sure care was safe and that any risks were monitored and managed. They also made sure people received consistent care, including when moving between different services.

Staff followed clear procedures when people were admitted to hospital. Important information about each person’s needs was shared with paramedics and hospital staff to help ensure safe and consistent care.

The team worked closely with GPs and other healthcare professionals to monitor people’s health and respond quickly if their needs changed.

When people had specific health conditions, staff used risk assessments to understand how to support them safely. The provider responded to changes in people’s needs and contacted or referred them to healthcare professionals when needed.

Safeguarding

The provider worked with people, their families, and healthcare professionals to understand what made people feel safe and how to support this. Staff focused on improving people's lives while making sure they were protected from bullying, abuse, discrimination, harm, and neglect. Any concerns were raised quickly and shared with the right professionals.

Staff received safeguarding training and knew how to recognise and report concerns. People told us they felt safe receiving care. Staff respected people's rights and put their safety first while supporting them to live fulfilling lives.

Any safeguarding concerns were fully investigated and reported to the local authority and the Care Quality Commission when needed. This helped to keep people safe and promote their wellbeing.

The provider followed the Mental Capacity Act 2005 and Deprivation of Liberty Safeguards to make sure decisions were made in people's best interests. People living at Stanton Manor were not subject to unnecessary restrictions and were supported to maintain as much independence as possible.

Involving people to manage risks

The provider worked with people to understand and manage any risks in all areas of their lives. Staff provided safe, personalised care that supported people to do the things that were important to them.

We saw staff working safely and helping people use equipment correctly and safely. Staff told us they had received training on how to support people to move safely using equipment, and training records confirmed this.

Staff were kept up to date about any risks or changes in people's needs through regular handovers and updates to care plans. They also discussed any risks related to people's health conditions with them.

Overall, the provider took a person-centred approach to managing risk, ensuring people received safe care while still being able to live independently and take part in activities that mattered to them.

Safe environments

The provider effectively identified and managed environmental risks. Equipment, facilities and technology supported the delivery of safe care.

Relatives told us they had no concerns about the environment. Staff said there were effective systems in place to manage risks and maintain a safe service. The environment was clean, tidy, and free from avoidable trip or slip hazards. Regular health and safety checks were carried out to identify, monitor, and address any potential environmental risks.

Safe and effective staffing

The provider made sure there were enough skilled and experienced staff, and that they received the training, support and supervision they needed to do their jobs well. Staff worked together effectively to provide safe, person-centred care.

There were enough staff on duty to meet people's needs. People told us they received support when they needed it and did not have to wait long for help. One person said, "The staff are very good and always around when I need them." During our visit, we saw staff regularly checking on people and responding quickly when assistance was needed.

People and their relatives said there were enough staff to provide the support required. Staff were confident in their roles, understood their responsibilities and told us they received regular training and supervision, which they found helpful and supportive.

Recruitment records showed that all necessary background and pre-employment checks had been completed before staff started work, helping to ensure suitable people were employed.

Infection prevention and control

The provider had clear systems in place to identify and manage the risk of infection.

Staff followed infection prevention and control procedures in line with current guidance. During our visit, we saw staff carrying out cleaning tasks when needed, helping to keep the service clean and hygienic.

Staff had access to the right personal protective equipment (PPE) and understood how to use it correctly. Feedback from both staff and relatives showed there were no concerns about infection control practices.

Medicines optimisation

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened.

People received their medicines as prescribed, including pain relief when needed and in line with advice from healthcare professionals. Medicines were stored securely, and regular checks were carried out to ensure they were managed properly.

Medication Administration Records (MAR) were completed correctly and showed that people received their medicines at the right times.

Staff knew how to give medicines safely and had completed the required training and competency checks. They also understood how to report and deal with any medication errors.
