Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response

to this.

People were supported to achieve good outcomes when accessing services to support their health needs. One relative told us staff had been slow to react to an aspect of their relative’s health needs in the past, but the present home manager worked with their team to address this and they felt staff were now being more proactive in ensuring their relative’s care was tailored to their needs. We saw feedback to the home manager about how the relative had praised staff’s quick response to a particular issue their relative had with continence and skin damage.

Staff showed good knowledge of the people in their care and what support they needed to make sure they received the care they needed. They told us how they worked with people, their relatives and external health professionals to achieve good outcomes for people. This included prompt actions to refer people to external health professionals when needed, such as the Speech and Language Therapy (SALT) team or their GP.
