Assessing needs
The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.
People’s needs were assessed to ensure staff provided the right support for them. Care records had good information on the levels of care people needed and nationally recognised assessment tools were used to assess people’s need. These included the Waterlow scoring tool that provided guidance for staff on reducing risk to skin damage for people which could lead to pressure ulcers. There was also good use of the Malnutrition Universal Scoring Tool (MUST) to support staff ensure people maintained a healthy weight.
Relatives told us they were involved with their family member’s care assessments and kept up to date regarding any changes to their needs.
Staff we spoke with told us the information in people’s care plans was up to date and reflective of people’s needs. All the staff we spoke with were able to access the electronic care system and told us any changes to people’s needs were highlighted and discussed at handovers and via alerts on the electronic system. Staff told us the communication between the different groups of staff was good. One staff member highlighted when there were any changes to a person’s dietary needs how this was communicated to both the kitchen staff and front of house staff who served meals to people.
