During an assessment under our new approach

Date of Assessment: 22-23 January 2025.

East Shore Partnership is a GP practice that provides a range of primary medical services at two sites, St Cuthberts Church, Hayling Avenue, Portsmouth, PO3 6BH, otherwise known as The Baffins Surgery and a branch site at Milton Road, Portsmouth, PO3 6DW, otherwise known as Milton Park Surgery.

The practice is situated within the NHS Hampshire and Isle of Wight Integrated Care Board (ICB) and delivers General Medical Services (GMS) to a patient population of approximately 14,000. The National General Practice Profiles states that demographics are in line with local and national averages. Information published by Office for Health Improvement and Disparities shows that deprivation within the practice population group is in the 5th decile (5 of 10). The lower the decile, the more deprived the practice population is relative to others. This assessment considered the demographics of the people using the service, the context the service was working within and how this impacted service delivery. Where relevant, further commentary is provided in the quality statements section of this report.

We undertook this assessment to check if improvements had been made following our previous inspection in March 2024. Since the last inspection, the practice had made improvements and is no longer in breach of regulations relating to Safe care and treatment, Good governance and Staffing.

The facilities and equipment met the needs of people, were clean and well-maintained with any risks mitigated. There were enough staff with the right skills, qualifications and experience. Managers ensured staff received training and regular appraisals to maintain high-quality care. Effective processes were in place for monitoring human resources, recruitment and staffing, including staff inductions, mandatory training compliance, and regular clinical supervision for non-medical prescribers with prescribing audits.

During an assessment under our new approach

East Shore Partnership is a GP practice that provides a range of primary medical services at two sites, St Cuthberts Church, Hayling Avenue, Portsmouth, PO3 6BH, otherwise known as The Baffins Surgery and a branch site at Milton Road, Portsmouth, PO3 6DW, otherwise known as Milton Park Surgery. East Shore Partnership is registered with the Care Quality Commission under the Health and Social Care Act 2008 to provide the following regulated activities; diagnostic and screening procedures, family planning, midwifery and maternity services, surgical procedures and treatment of disease, disorder or injury. This announced assessment was undertaken between 11 and 14 March 2024. We carried out our on-site visit on 12 March 2024. We undertook this assessment in response to concerns we received about the practice’s hot water systems and staff culture. During our assessment, we found concerns around the management and safety of the hot water systems, the risk of exposure to legionella as well as the risk of exposure to infections as suitable records of staff immunisations had not been maintained. We found a lack of effective governance systems and processes to assess, monitor and improve the quality and safety of the service. There was a lack of regular documented clinical supervision for non-medical prescribers instigated by supervisors, including audits of prescribing. We identified gaps in recruitment records for staff members we reviewed during our on-site visit. Clinical competencies were not obtained at the time staff were recruited to ensure staff were suitably qualified for the role. These concerns resulted in 3 breaches of regulations in relation to safe care and treatment; staffing and good governance. However, we found staff were able to raise concerns and there were processes in place for the practice to investigate and appropriately respond to complaints. You can find more details of our concerns in the evidence category findings.

During a routine inspection

This practice is rated as Good overall.

The key questions at this inspection are rated as:

Are services safe? – Good

Are services effective? – Good

Are services caring? – Good

Are services responsive? – Good

Are services well-led? - Good

We carried out an announced comprehensive inspection at East Shore Partnership on 12 June 2018 as part of our inspection programme.

At this inspection we found:

  • The practice had clear systems to manage risk so that safety incidents were less likely to happen. When incidents did happen, the practice learned from them and improved their processes.
  • The practice routinely reviewed the effectiveness and appropriateness of the care it provided. It ensured that care and treatment was delivered according to evidence- based guidelines.
  • Patients who were frail were supported by an outreach matron and pharmacist, employed by the practice.
  • Staff involved and treated patients with compassion, kindness, dignity and respect.
  • There was a strong focus on continuous learning and improvement at all levels of the organisation.

The areas where the provider should make improvements are:

  • Review the safe storage of medicines which require refrigeration.
  • Review access to appointments to improve patient satisfaction.
