Our view of the service
Date of assessment: 11 March 2025 to 04 April 2025. Gracelands Care Home is a care home providing accommodation and personal care for older and younger adults some of whom may be living with dementia, a learning disability or autism. They are also registered to support people with a physical disability, sensory impairment, mental health need or those detained under the Mental Health Act. The home is registered to accommodate 43 people. At the time of the assessment there were 34 people living at the home. At the last inspection in July 2024, we rated the home good.
This assessment was completed due to concerns in relation to staffing and risk management. We assessed a small number of quality statements from all key questions focusing on the areas of concern. The assessment did not indicate any concerns, and the overall rating remains good.
The management team and staff worked well together to create a positive culture focused on the people receiving care. Safe recruitment practices were followed. Staff were well supported and appropriately trained. People’s needs were assessed, and care plans and risk assessments were in place which were regularly reviewed and updated.
Accidents, incidents and safeguarding concerns were reported promptly. Appropriate action was taken to learn, improve care and minimise any future risks.
The principles of the Mental Capacity Act (2005) were understood and implemented. Capacity assessments and best interest decisions were recorded where needed.
A quality assurance system was in place which gave robust oversight and supported the management team to identify any areas for development which were quickly acted upon.
We have assessed the service against ‘Right Support, Right Care, Right Culture’ guidance to make a judgement about whether the provider guaranteed people with a learning disability and autistic people respect, equality, dignity, choices, independence and good access to local communities that most people take for granted.
People's experience of this service
People were supported by staff who knew them well, were kind and compassionate, and understood their health and wellbeing needs and wishes.
Feedback from people and relatives was wholly positive in relation to feelings of safety. We were told staff were very good and always checked on people to make sure they were okay. Comments included, “I like it here and I feel safe, they do anything you ask them” and “I like being here the staff look after you and that’s what’s important.”
People were engaged in meaningful activities and were supported to maintain relationships. Visitors were allowed at any time and although protected mealtimes were in place family members and friends were encouraged to have meals with people. One person said, “Everybody is friendly I can do what I want, time seems to go fast, we are lucky to find a place like this,” and another person commented on the “thoughtful nature” of the staff.
People were involved in all aspects of their care and individual choices and preferences were known and respected.
People, and their relatives told us the management team were very approachable, and they felt able to raise concerns if they needed to.
