At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s needs were met through good organisation and delivery.

Person-centred Care

Score: 3

The provider made sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in people’s needs. Care plans were personalised, and people and relatives told us staff provided person-centred support. A range of activities took place that reflected people’s hobbies and interests. One person told us, “The carers are excellent, just like a family atmosphere.”

Care provision, Integration and continuity

The provider understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity. People at the service had formed a close-knit community, which was well-integrated into the wider area. People and staff had professional but close relationships, and in many cases had known each other for many years. One person said, “The care is excellent and the carers are… totally dedicated to help.”

Providing Information

The provider supplied appropriate, accurate and up-to-date information in formats that were tailored to individual needs. People’s communication needs were assessed and monitored, which allowed staff to give them information in the best ways possible and help them make their voices heard.

Listening to and involving people

The provider made it easy for people to share feedback and ideas, or raise complaints about their care, treatment and support. Staff involved people in decisions about their care and told them what had changed as a result. Feedback was regularly sought, including through meetings, and people and relatives said this was acted on. One relative told us, “They appear very responsive to ideas.”

Equity in access

The provider made sure that people could access the care, support and treatment they needed when they needed it. Healthcare partners did not raise any concerns about how staff supported people to access care and support when needed.

Equity in experiences and outcomes

Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this. Staff knew people and their needs well and used this knowledge to help people achieve positive outcomes.

Planning for the future

Score: 3

People were supported to plan for important life changes, so they could have enough time to make informed decisions about their future, including at the end of their life. Systems were in place to ensure people’s wishes about their future care were sought and documented.
