Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people were safe and protected from avoidable harm.

Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learned to continually identify and embed good practice. The provider had systems to investigate and act on incidents, accidents, safeguarding concerns and complaints. Outcomes were shared with relevant parties, where needed, in line with duty of candour requirements.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services. A relative said, “[Registered manager] really helped with the moving in process.”

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately. Staff knew about the safeguarding process and were confident to raise concerns, if needed. People and staff both felt the home was a safe place to live. A person told us, “The carers will check in on me regularly to see if I am okay.” Deprivation of Liberty Safeguards (DoLS) authorisations were in place for all relevant people, and any conditions were added to people’s individual care plans.

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them. Where potential risks had been identified, a risk assessment had been carried out. This provided personalised information for staff about how to help keep people safe.

Safe environments

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care. The provider completed environmental risk assessments and health and safety checks to help keep the environment safe. Equipment was serviced regularly to ensure it was working correctly.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs. There were enough staff deployed to meet people’s needs appropriately. One person told us, “The staff will come quickly (if I push the call bell). The staff are lovely."

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. Staff had been trained in infection control and prevention (IPC) best practice and followed these principles when delivering care. People confirmed this was the case and commented positively on the cleanliness of the home. A person said, “It's clean and nicely decorated.”

Medicines optimisation

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened. People received their medicines when they were due. The provider had improved the admission process, following a serious incident with medicines when someone was admitted to the home from hospital. This included improving the guidance for staff to follow, additional quality checks and specific staff training.
