Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence.

We assessed a limited number of quality statements in the effective key question and found areas of good practice. The scores for these areas have been combined with scores based on the rating from the last inspection. This key question has been rated good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.

People’s needs were assessed and met through detailed care planning. Assessments covered a wide range of areas, including physical and mental health, behaviours, communication and personal care.

People and their relatives took part in assessments and regular reviews of their care and support. When required, specialist assessments were completed by health and social care professionals, and their guidance was included in people’s care plans.

There was an ongoing programme to update and strengthen care plans so they contained rich detail about people’s needs and choices.

We looked at how the service supported people to make their own decisions and sought consent in line with the Mental Capacity Act 2005 (MCA). Staff understood the importance of gaining consent, but there were inconsistencies in their knowledge and understanding of mental capacity and best-interest processes. Records did not always clearly show the rationale for specific restrictions or how these were assessed and agreed.

The provider identified these gaps and taken initial steps to strengthen staff understanding through additional guidance and support. Further work was planned, including forums and support sessions, to improve staff knowledge and embed more consistent practice across the service and organisationally.
