The service showed a positive safety culture with effective incident reporting, learning, and high training compliance, supporting safe patient care and clear care pathways. Staff felt confident raising concerns, and environments, equipment, and infection control were generally well managed. However, there were notable gaps in governance, including outdated or inaccurate policies, insufficient safeguarding training oversight, and low appraisal compliance. While risks were largely controlled and patients kept safe, improvements are needed in documentation accuracy, staff development, and overall governance arrangements.
