Well-led – this means we looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture. At our last assessment we rated this key question requires improvement. At this assessment the rating has changed to good. This meant the service was consistently managed and well-led. Leaders and the culture they created promoted high-quality, person-centred care.

Shared direction and culture

Score: 3

The provider had a shared vision, strategy and culture. This was based on transparency, equity, equality and human rights, diversity and inclusion, engagement, and understanding challenges and the needs of people and their communities. The provider evidenced an improved understanding person‑centred care and this was supported by improved care‑planning processes that provided staff with more detailed guidance on how to provide safe care. We also found improvements had been made to the systems in place to analyse and action accidents and incidents which meant the provider had a better oversight of the service.

Capable, compassionate and inclusive leaders

The provider had inclusive leaders at all levels who understood the context in which they delivered care, treatment and support and embodied the culture and values of their workforce and organisation. Leaders had the skills, knowledge, experience and credibility to lead effectively. They did so with integrity, openness and honesty. The provider had made improvements to the systems and processes in place to ensure risks to people’s safety were effectively monitored and mitigated. The provider demonstrated a good understanding of person-centred care and ensured staff had appropriate guidance to follow.

Governance, management and sustainability

Score: 3

The provider had clear responsibilities, roles, systems of accountability and good governance. They used these to manage and deliver good quality, sustainable care, treatment and support. They acted on the best information about risk, performance and outcomes, and shared this securely with others when appropriate. The provider had made improvements to ensure there was effective management oversight of the service. Regular audits were carried out to monitor all aspects of service delivery, and these were reviewed and analysed by the registered manager and the provider. Findings from the audits were used to drive service improvement and were shared with the wider staff team. The provider had also made significant improvements to how people’s care was assessed, planned and reviewed, as well as to the reporting and management of safeguarding incidents.

Learning, improvement and innovation

Score: 3

The provider focused on continuous learning, innovation and improvement across the organisation and local system. They encouraged creative ways of delivering equality of experience, outcome and quality of life for people. They actively contributed to safe, effective practice and research. Systems and processes to support the effective assessment, monitoring and mitigation of risks to people’s safety had been strengthened since our last inspection. We found clear and consistent arrangements in place that supported learning and improvement across the service. The provider had also invested in new electronic systems to enhance their oversight and ensure information was recorded, monitored and acted upon in a timely and reliable way.
