Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence. At our last assessment we rated this key question requires improvement. At this assessment the rating has changed to good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. Initial assessments of people’s needs had been completed before they started using the service. Since our last inspection, the provider had strengthened the quality of their care planning. Care plans now clearly set out the level of support people required, including detailed guidance on the mobility equipment they used, how staff should check equipment was safe, and the safe techniques staff should follow to meet people’s individual needs.

How staff, teams and services work together

Score: 3

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. Following our last inspection, the provider had reviewed and updated people’s care plans and risk assessments. We found information from external professionals had been used to update these documents, ensuring staff had clear and current guidance to support people safely. People and their relatives provided us with positive examples of how the provider had worked with other services to meet people’s needs. For example, the provider had ensured people were assessed and provided with equipment to aid their mobility.

Monitoring and improving outcomes

Score: 3

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves. The provider had made improvements to how they assessed and monitored peoples known risks. Care plans provided staff with clear and detailed guidance to ensure people’s outcomes were monitored and met. For example, where people had a history of falls, their care plans now included specific instructions for staff on how to reduce this risk. Care plans and risk assessments had been regularly reviewed and updated to reflect people’s current needs, helping to ensure they were supported safely and reducing the likelihood of avoidable harm.

 

 

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. The provider had made improvements to how people’s capacity to consent to care was assessed. We found capacity assessments in place for all relevant aspects of care that people received ensuring decisions were made in line with the principles of the Mental Capacity Act. This helped ensure people’s rights were protected and that care was delivered in a lawful and person‑centred way.
