Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this.
The provider delivered a person-centred care approachwhere individual needs, preferences, and goals were prioritised, ensuring support was tailored to maintain their quality of life, including hobbies, relationships, and personal aspirations.
A healthcare professional told us how the service had advocated for a person to receive additional funding to make significant adaptions to their home, which had a positive outcome on their wellbeing. They told us, “We put them forward for a national award for this as their approach was brilliant.”
