People were supported to overcome any difficulties they might encounter to make sure they experienced equality in care. Staff made reasonable adjustments to adapt the care they provided to meet people’s individual needs. They developed and maintained strong links with other health and social care professionals so they could access support and advice on how to minimise potential obstacles people faced due to their disability.

People and relatives told us the provider focused on positive experiences and outcomes for individuals. Staff worked with people to identify barriers people experienced and introduced personalised ways of overcoming them in the care and support they provided. For example, the registered manager provided additional support to one person so they could attend a local community activity to reduce the risk of them becoming isolated.

Care plans were person-centered and tailored to people's individual needs and wishes. The provider assessed what format people preferred records in and adapted this according to their needs.
