Well-led – this means we looked for evidence service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture. This is the first assessment for this service. This key question has been rated good. This meant the service was consistently managed and well-led. Leaders and the culture they created promoted high-quality, person-centred care.

Shared direction and culture

The provider had a shared vision, strategy and culture. This was based on transparency, equity, equality and human rights, diversity and inclusion, engagement, and understanding challenges and the needs of people and their communities. The leadership team demonstrated an understanding of regulatory requirements and was undergoing a period of auditing following a change in management to understand where to focus resources within the service. The provider had implemented an action plan and was proactively following this to strengthen areas within the service, such as staffing and medicines management.

Capable, compassionate and inclusive leaders

The provider had inclusive leaders at all levels who understood the context in which they delivered care, treatment and support and embodied the culture and values of their workforce and organisation. Leaders had the skills, knowledge, experience and credibility to lead effectively. They did so with integrity, openness and honesty. There had been a recent change in management at the service, and the leadership team was working actively to engage with staff to understand their views of the service and support needs. The provider recognised, in some areas, staffing levels were maintained through existing staff covering shifts which had created stress and difficulty for some. The provider had recruited additional staff to alleviate pressure in these areas.

Freedom to speak up

The provider fostered a positive culture where people felt they could speak up and their voice would be heard. Overall staff told us they knew how to raise concerns and who to contact if needed. The provider had clear policies in place regarding whistleblowing. Records evidenced where people had raised concerns about care delivery, these were addressed quickly.

Workforce equality, diversity and inclusion

The provider valued diversity in their workforce. They worked towards an inclusive and fair culture by improving equality and equity for people who worked for them. Policies and procedures were in place regarding equality, diversity and inclusion. There were no concerns regarding equality, diversity or inclusion were raised by staff during the assessment.

Governance, management and sustainability

Score: 3

The provider had clear responsibilities, roles, systems of accountability and good governance. They used these to manage and deliver good quality, sustainable care, treatment and support. They acted on the best information about risk, performance and outcomes, and shared this securely with others when appropriate. Clear governance structures were in place to continually monitor the quality and effectiveness of the service. Additional supervisory staff had been recruited to support with auditing across the service.

Partnerships and communities

The provider understood their duty to collaborate and work in partnership, so services worked seamlessly for people. They shared information and learning with partners and collaborated for improvement. Professional feedback regarding the service was positive. Partner agencies told us the provider was easy to contact and worked collaboratively to share relevant information in a timely manner.

Learning, improvement and innovation

The provider focused on continuous learning, innovation and improvement across the organisation and local system. They encouraged creative ways of delivering equality of experience, outcome and quality of life for people. They actively contributed to safe, effective practice. The provider ensured processes were in place to record and learn from accidents and incidents. Where safety events had occurred, the leadership team undertook analysis of themes and trends and recorded what had worked to mitigate future risks. The service had identified areas where they wanted to strengthen their processes and had implemented action plans and ongoing assessments to ensure continuous service improvement.
