Person-centred Care

The provider made sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in people’s needs. Staff and leaders at the service were able to quickly identify and address concerns as they arose. People told us they felt confident speaking to staff if they had any issues. A relative told us, “I spoke with staff when my relative deteriorated and needed extra assistance. This was put in place.”

Care provision, Integration and continuity

The provider understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity. Records evidenced that where required, professional guidance and advice was sought to support people and this information embedded within people’s plans of care. The service communicated effectively with partner agencies and had systems in place to actively monitor people’s well-being.

Providing Information

The provider supplied appropriate, accurate and up-to-date information in formats that were tailored to individual needs. People’s communication needs and preferences were assessed and documented within care plans, including information about aids and adaptations people used. People and their relatives told us it was easy to contact the service if they needed anything.

Listening to and involving people

The provider made it easy for people to share feedback and ideas, or raise complaints about their care, treatment and support. Staff involved people in decisions about their care and told them what had changed as a result. People and their relatives told us they were involved in in the care planning process and subsequent reviews. This ensured people’s choices were fundamental in shaping their care and support.

Equity in access

The provider made sure that people could access the care, support and treatment they needed when they needed it. Care was delivered in line with people preferences. People could access support and advice from health and social care professionals where needed, and the provider ensured people were supported with this where required.

Equity in experiences and outcomes

Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this. Records evidenced that where required, the provider had worked to ensure health and social care professionals were involved in peoples care and support. Where issues were identified, the provider worked to escalate concerns to the correct team of people.

Planning for the future

People were supported to plan for important life changes, so they could have enough time to make informed decisions about their future, including at the end of their life. People and their relatives had the opportunity to discuss end of life wishes should they wish to do so, and staff received training how to support people in line with their preferences.
