This is the first assessment for this service. Our overall rating of safe at HTG Norwich was inadequate.

The service did not consistently promote a proactive approach to safety. Staff concerns were not always acknowledged, thoroughly investigated or addressed, and opportunities for learning from safety events was limited. Systems intended to manage risk, ensure continuity of care, infection control, and ensure appropriate staffing were not reliably effective. As a result, people’s safety, rights, and individual needs were not always prioritised or protected.
