The service did not consistently deliver effective care and treatment. People’s needs and preferences were not always assessed, discussed or incorporated into care planning. Monitoring of outcomes and coordination across teams was inconsistent, and transitions between services were not always well managed. Although consent was respected, person-centred care was not reliably delivered.
