Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice. No safety events, accidents or incidents had occurred at the service, processes were in place to record and analyse information from accidents and incidents should these occur. The registered manager demonstrated a thorough understanding of how to learn from safety events.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services. The registered manager and staff understood how to work alongside health, social care and community services. Records were up to date and available to be shared with professionals when required.

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately. Staff received training in safeguarding both adults and children, staff demonstrated an understanding of how to identify, address and report safeguarding concerns should these occur. A person told us, ‘I feel safe with the staff.’

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them. People were involved in creating their care plans so their preferences for support were recorded and followed.

Safe environments

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care. The provider undertook an assessment of the care environment prior to the commencement of care to ensure safety for both the person and staff members. This included recording any equipment used within people own homes and who was responsible for servicing this equipment. Staff received training in how to safely use equipment and adaptations and visual checks of the safety of equipment were undertaken.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs. Staff were recruited safely with all required suitability checks taking place before commencing employment. This included criminal record checks, references from past employers and competency-based interview. Staff received an induction, training and ongoing competency assessments to ensure their skills met the needs of people.

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. Staff received training in how to work in line with infection prevention and control guidelines and had access to adequate stocks of Personal Protective Equipment (PPE), such as gloves and aprons.

Medicines optimisation

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened. Medicines were managed safely. The service accurately recorded medicines administration and ensured staff received ongoing training and checks of their competence in relation to medicines administration.
