Person-centred Care
The provider ensured people were consistently at the centre of their care and support.
Decisions about their care and support was delivered flexibly to reflect their wishes, preferences and changing needs. Care plans reflected individual preferences and were used to guide individualised support. Staff described enabling people to lead their routines and make decisions about their activities and lifestyles.
Care provision, Integration and continuity
The provider ensured care was coordinated, consistent and supported continuity for people. People were supported by regular staff teams, which enabled strong relationships and continuity. Staff worked closely with professionals to ensure care remained joined-up and responsive.
Providing Information
The provider supplied appropriate, accurate and accessible information in formats tailored to individual needs. Communication plans detailed how to support people effectively, including the use of assistive technology and communication tools. This supported people to understand and engage in decisions about their care.
Listening to and involving people
The provider made it easy for people and relatives to give feedback and be involved in decisions. People confirmed they could raise concerns and felt listened to. Staff described an open culture where feedback was encouraged and used to improve care. One staff member said, “We are encouraged to go to the manager with recommendations. For example, a solution was shared and is now working practice; improving daily activities.”
Equity in access
The provider made sure that people could access the care, support, and treatment they needed when they needed it. People were supported to access activities, services, and experiences when they wanted. Staff described supporting holidays, and community access and social activities demonstrated people could access services and activities without unnecessary restriction. A relative added, “He went to Spain last year which was a fantastic time for all of us.”
Equity in experiences and outcomes
Staff and leaders tailored support to ensure equitable outcomes. Care reflected people’s individual needs, including communication and physical health requirements, and aimed to provide consistent, positive experiences.
Planning for the future
The provider supported people to plan for future care needs. An advance care planning template had been introduced to capture people’s preferences, including for future treatment and care. This ensured staff and professionals could respond appropriately if people’s needs changed.
