The provider identified their main aim and direction was to prioritise patient experience and continually strive for improvement. The provider had a shared vision, strategy and culture. This was based on transparency, equity, equality and human rights, diversity and inclusion, engagement, and understanding challenges and the needs of people who use the service.

Governance, management and sustainability
Governance systems did not ensure that records of care and treatment were always completed in line with the provider’s own policy. Medical histories and current medicines did not match across the provider’s patient questionnaires, and from the sample of consultation records we reviewed, we noted several inaccuracies.

The provider focused on continuous learning, innovation and improvement across the service. They encouraged creative ways of delivering equality of experience, outcome and quality of service to the people who chose it. The provider actively contributes to safe, effective practice and research. The provider attended and contributed to industry-led conferences to ensure they were informed about new innovations and also updated about changes to established procedures.
