We served an urgent Notice of Decision on 25Eight Medical Group Ltd on 30 July 2025 to suspend the regulated activity of transport services, triage and medical advice provided remotely due to significant concerns around service user's safety and breaches of five regulations of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014 at Swadlincote Station.

During an assessment of Emergency and urgent care

We carried out an onsite inspection of Swadlincote Station on 28 July 2025. The service was registered with the Care Quality Commission in July 2024 to provide the regulated activities of transport services, triage and medical advice provided remotely, and treatment of disease, disorder or injury.

This inspection focused on the Emergency and Urgent Care Assessment service group and was prompted by information received concerning the safety of the service.

We found breaches of the legal regulations in relation to safe care and treatment, premises and equipment, governance and staffing.

However, we found leaders were responsive to feedback and committed to improving. The service worked well with stakeholders and demonstrated a commitment to working in partnership with external organisations to support safe and effective care delivery at events.

During an assessment of the hospital overall

Date of inspection: 28 July 2025. Swadlincote station registered with CQC in July 2024 to deliver the regulated activities of transport services, triage and medical advice provided remotely and treatment of disease, disorder or injury. At this inspection we inspected the Emergency and Urgent Care assessment service group. This inspection was carried out in response to information of concern we received about the service, and in relation to people’s safety.

During this inspection we identified breaches of the following regulations: Regulation 12: Safe care and treatment, Regulation 15: Premises and Equipment, Regulation 17: Good Governance, Regulation18: Staffing and Regulation 19: Fit and proper persons employed. In instances where CQC have decided to take civil or criminal enforcement action against a provider, we will publish this information on our website after any representations and/ or appeals have been concluded.

Following this inspection we rated the service inadequate and this service is being placed in special measures. The purpose of special measures is to ensure that services providing inadequate care make significant improvements. Special measures provide a framework within which we user our enforcement powers in response to inadequate care and provide a timeframe within which providers must improve the quality of the care they provide.
