Person-centred Care
The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice.
The provider had an accidents and incidents log in place. The provider said they discussed lessons learnt with staff if any incidents occurred. We saw evidence of this through team meetings.
Care provision, Integration and continuity
The provider understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity.
Your life worked alongside other health and social care professionals to ensure they met people’s needs. There was evidence in care plans of involvement from other health professionals.
Providing Information
The provider supplied appropriate, accurate and up-to-date information in formats that were tailored to individual needs. Care plans detailed people’s communication needs these were up to date at the time of assessment.
Listening to and involving people
The provider made it easy for people to share feedback and ideas, or raise complaints about their care, treatment and support. Staff involved people in decisions about their care and told them what had changed as a result.
The provider had processes in place to manage information of concern or complaints.People and their relatives told us they were aware of how to raise a complaint. One staff member told us, “I would record anything that I saw or was told , and would report to management, and check on the next visit if anything had been done, if not, I would escalate by whistleblowing or contacting the Care Quality Commission.”
Staff told us they had team meetings where they could share ideas or raise concerns. A relative told us, “We have submitted feedback informally and participated in care reviews. We are also in ongoing communication with the estate manager and Wakefield Social Services as part of the care planning and transition process.”

Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this.
The provider had policies, training, and systems in place to encourage people and staff to speak up about equality. There was a whistleblowing policy in place encouraging people and staff to speak up.
