Learning culture

Score: 3

The provider had a positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice. Regular meetings and feedback mechanisms supported continuous improvement. Staff described a supportive environment where feedback was acted upon.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services.

People told us staff worked with health and social care professionals to ensure any equipment they needed was appropriate and used safely.

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately. Staff understood their safeguarding responsibilities and knew how to report concerns. Records of safeguarding referrals and notifications were maintained.

People told us they felt safe with the support provided by staff and would feel confident to raise any safety concerns with managers. One person said, “I have never had a problem, the staff know what they are doing, if I had a problem I know I could depend on them to sort it out.”

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them. In response to a recent local authority visit, managers were in the process of updating care records to ensure they were more detailed, and person centred. Care plans and risk assessments we reviewed were person-centred, but the process of updating was still on going.

People were actively involved in risk management and care planning, and peoples’ preferences were respected. People told us that risks were well managed. One relative said, “When I go [out] I am confident my [person] won’t fall. They keep an eye on [person].”

Safe environments

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care.

Risk assessments for people’s homes were in place. One person told us, “The staff check that all my equipment is in the right place, they will point out anything that is wrong and needs repairing.”

Safe and effective staffing

Score: 3

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs. Visits were well organised, ensuring continuity and flexibility to meet individual needs. All required recruitment checks were in place and staff had access to comprehensive training and regular supervision and support.Staff told us the training was good, and they felt supported.

People told us staff were competent, hardworking, reliable and punctual. People said, “They [staff] always arrive bang on time”, “The team are fantastic, I cannot fault them” and “I can’t fault them, new staff always come, for the first 2 visits, with an experienced member of staff that I know. I have never had a problem.”

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. Infection prevention and control training was up to date. Spot checks confirmed compliance with hygiene standards. PPE was available for staff and used appropriately.

People told us staff has good hygiene practises they said staff left their homes clean and tidy.

Medicines optimisation

Medicines were administered safely overall, but some documentation required improvement. Some medicines administration records (MAR) lacked clarity for ‘as required’ and variable doses medicines, refusal codes were not always explained, and stock counts were missing. The registered manager showed us that a new system for MAR reviews, audits and weekly stock checks had been put in place.
