Person-centred Care

Score: 3

People received person-centred care which met their needs and reflected their preferences. Relatives told us the agency was responsive and flexible, making changes whenever they needed. Care plans were appropriately detailed and showed how people liked to be cared for.

Care provision, Integration and continuity

Staff liaised with other professionals involved in people’s care when needed. This helped to ensure people received continuity of care.

Providing Information

The provider gave people a range of information about the service and copies of their care plans. Relatives told us staff kept in touch and informed them about anything they needed to know.

Listening to and involving people

The registered manager regularly contacted people using the service by telephone and in person to ask for their views about the service. People were asked to complete surveys about their experiences.

 

There was a suitable complaints procedure. Relatives explained they knew what to do if they had any concerns or complaints. A relative told us that whilst they had not had any complaints, they had raised a query with the registered manager. They told us this was responded to immediately and to their satisfaction. Another relative commented, “I can contact the manager if I need to. [They] are in regular contact with me and we can discuss anything we need.”

Equity in access

People were able to access the care and support they needed in a way that worked for them.

Equity in experiences and outcomes

People had equity in experience and outcomes. Staff undertook training about equality and human rights. Care plans included information about people’s personal history, interests and goals.

Planning for the future

Staff worked with people and their relatives to identify any future plans they had. They helped people to retain skills and independence when they were able to.
